Texas Home Health is seeking an intake LVN to join our home health team.  The i
ntake coordinator 
is responsible for facilitating the referral process and the coordination of information to patients and their family members, physicians, facility staff, home health agency staff, and others involved in assuring continuity of patient care.  As part of this team, the intake coordinator will:
Receive referrals and advise patients, family members, staff, and physicians about programs available for patient care needs.
Document referral orders with signature and date; ensures orders received by LVN/LPN are reviewed and cosigned by RN.
Comply with the applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program:  Code of Conduct, HIPAA and Documentation Standards.

Qualified candidates will have the following:
Registered Nurse or Licensed Vocational Nurse with current state license.
Minimum of one (1) year experience in home care preferred.
Prior use of Electronic Medical Records (EMR) preferred.
Knowledge of state, federal, and local regulatory requirements.
Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.  Ability to travel to all business locations.
